 You can continue to purchase Aflac insurance protection on a direct basis at home.   These are your 3 different payment options:

1.  Monthly billing automatically (worry-free) paid from your credit card. You will need Acrobat Adobe Reader (free download) on your computer in order to download, view, and print the forms you will need.  Click HERE to download the FREE Adobe Reader software.  If you want to pay for your Aflac coverage with your credit card each month, fill out the form below (click button) and fax it to: 254 554-2761 for immediate processing.  If you aren't sure (or don't know) what policies you have or the policy numbers, call 254 554-0909 in Killeen for assistance.

2.  Monthly billing automatically (worry-free) drafted from a local valid checking account. If you want Aflac to draft your checking account each month to pay for your Aflac coverage, or fill out the form below and fax it to: 254 554-2761.  Aflac will draft your bank account each month. If you aren't sure (or don't know) what policies you have or the policy numbers, call 254 554-0909 in Killeen for assistance.

3.  Monthly invoice paid directly to Aflac by check (invoiced at home address).   You will receive a monthly invoice from Aflac headquarters and you'll be responsible for paying on time by check. No form needed, you will receive an invoice from Aflac headquarters shortly.

 

If you want to make any changes on your Aflac plans:

  • change of address
  • add or delete a family member
  • change in beneficiary

If you do not want to continue your Aflac coverage, you may also cancel at any time after December 1, 2006.  If you wish to cancel any or all of your Aflac plans, fill out the form below and fax it to: 254 554-2761.  Coverage will end the last day of the month the notice is submitted. You cannot cancel your Aflac coverage by calling your PCSI corporate office in Austin.  If you aren't sure (or don't know) what policies you have or the policy numbers, call 254 554-0909 in Killeen for assistance.

PLEASE NOTE: DO NOT contact the PCSI corporate office in Austin regarding any Aflac issues, they will only refer you back to the agent office.

You may call the Aflac agents directly at: 254 554-0909 or 254 458-4293.  If you reach voicemail, PLEASE TELL US THE FOLLOWING:

  • Full name and city
  • Duties (KP, Warehouse, etc.)
  • Work location
  • Contact phone number

PCSI's decision to no longer payroll deduct Aflac premiums in the future will NOT interfere with your family's valuable Aflac protection as long as you inform us on how you wish to continue paying your premiums.  Your Aflac coverage rates will NOT increase and your coverage is guaranteed as long as premiums are paid.
